Scholarship entrants must provide the following:

• Introductory letter that includes:
  • Student name and school they are graduating from
  • Name of parent/legal guardian and the facility and department they work in
  • Where they have applied to for post-secondary education of where they will be attending (if known)
• Copy of January transcipt marks
• Copy of letter of acceptance or conditional acceptance from the post-secondary educational institute
